From 24-hour care and supervision and medication management to socialization and companionship service, you can trust us to do it with the utmost respect and compassion to your loved one’s situation; offered in a clean, safe, pleasant, and the least restrictive environment possible.

All of our services are provided according to the resident’s wants and needs; based on their pre-admission assessment and along with our basic needs and services plan. Subsequent resident appraisals may result in the need for additional basic services.

Here is more of what you can expect from Heartland Adult Residential Facility.

Basic services at a minimum include, but are not limited to the following:

  • Continuous care and supervision 24/7
  • Monitoring for changes in physical, mental, emotional, and social functioning, and changes in medical needs
  • Notification to the resident’s family, physician, and other appropriate person/agency of resident’s needs and status
  • Lodging

Food Services include:

  • Three nutritious meals daily and snacks
  • Other meal services such as vegetarian or other dietary preference based on the resident’s physician’s orders
  • Assist resident with cooking, preparing, and serving three balanced meals and healthy snacks daily

Additional services cover:

  • Helping gain access to supportive services specified by IPP.
  • Assist resident with cleaning bedrooms daily for general cleaning and weekly (or more if needed) for cleaning of bedding, towels, linen blankets, and pillows. Provide residents with personal hygiene items such as shampoo, soap bars, toothpaste, toothbrushes, deodorants, hair gels, hair combs, and shaving materials for each resident.
  • Assist with laundry services that include washing, ironing, and mending as needed.
  • Assist with self-care, hygiene, and grooming as needed (dressing, bathing, showering, toileting, shaving, brushing teeth, combing hair, getting haircuts, trimming nails, and all other personal hygiene needs).
  • Transportation to and from medical appointments, recreational activities, and other engagements.
  • Scheduling of medical care with primary physicians, dentists, physician specialists, and mental health specialists.
  • Assist and participate in the resident’s IPP/ISP meetings and needs, as well as in their respective Emergency Plan, which includes regular home safety and preparedness checks
