Know if you are qualified for our assisted living arrangement
Is Heartland Adult Residential Facility the right home for your loved one? They must be assessed first before becoming accepted as a resident. We welcome individuals who are:
- Male or female
- 18 to 59 years of age who are intellectually delayed or have physical disabilities
- Disabled adults requiring living arrangements that accommodate their medical healthcare needs
- Needing support with activities of daily living (ADLs)
Application information shall include the following:
- Admission Agreement
- Refund Policy
- Current IPP
- Needs and service plan
- Functional capabilities assessments
- Information regarding individual likes, dislikes, abilities, interests, and preferred activities
- History of an aggressive or dangerous behavior of the potential resident towards their own self and/or others
- Identified needs for training and treatment activities
- Identified medical needs, including dietary requirements
- Name, address, and telephone number of the consumer’s authorized representative
- The name, address, and telephone number of actively involved parents or relatives
